An insurance claim is the actual application for benefits provided by an insurance company. Policyholders must first file an insurance claim before any money can be disbursed. In making a claim there are procedural forms required based on the nature of the claim. In order to provide hassle-free service whenever there is a Life Insurance Claim, please be aware of the required procedure to alleviate stress since this is always an emotional time. DEATH CLAIMS The Claimant is the named beneficiary and trustee, trustee, next of kin or assignee in the case of absolute assignments. If the beneficiary is Estate, then Letters of Administration or Probate of Will naming the person to whom benefits should be paid is required. The process is 4 simple steps. Step1 Claimant informs Maritime Life of the death of the insured Step 2 Claimant provides Maritime Life with the following information. This can be provided over the phone. - Date of death of the insured - Claimant’s mailing address - Claimant’s contact telephone number - Relationship to insured/deceased Step3 Claimant submits three forms. - Statement of Identity. This must be filled out by someone who was acquainted with but not related to the deceased, and who has seen the body - Claimant’s Statement filled out by the claimant or assignee (where applicable) - Attending Physicians Statement filled out by the regular physician or doctor who last attended to the insured or pathologist who performed the autopsy. Step4 Claimant provides Maritime Life with: -Death Certificate -Proof of Age -Policy Contract For Suicide and Accidental Death Claims the procedure is the same except with one additional requirement: THE CORONER’S REPORT Dread Disease or Critical Illness Claims (on OLNP, Triflex) 1.Claimant informs Maritime Life of the diagnosis of a dread disease 2.Claimant submits two forms a) Claimant’s statement filled out by claimant b) Attending Physician’s statement filled out by the physician who made the diagnosis 3.
