The claimant is the Admed insured. The Attending physician or surgeon is the Doctor who attended to the insured.
For Hospital Diagnostic Claims
a) The insured seeks prior approval from the Admed Unit before any tests are performed to ensure full payment under this benefit. Without approval, the benefit is limited. Prior approval consists of a note from the attending physician stating:
–The Insured’s symptoms
-The name of the tests to be performed
The Claimant also provides the original bill or receipt
Surgical Claims:
The insured informs the Admed Unit of the exact name of the surgical procedure to be performed. This is needed to:
-Ensure that the procedure is covered
-Determine the category under which the surgery falls
The claimants and physician’s forms are filled out and submitted along with proof of age.
For Accident and Emergency Claims
The claimant notifies the Admed unit as soon as possible. A message can be left on the Admed answering machine after working hours and on weekends.
The claimant and physician form must be submitted to process the claim.
